Incident Reporting

Critical Incident and Unusual Incident Reports are used to report any unusual incident and actions taken by Employees, Contractors, Subcontractors and Volunteer at your program. Critical and Unusual Incidents should be reported in within 24 hours to TCP and the DHS (when applicable). Although the Critical Incident and Unusual Incident Reports detail similar information, which form to be used will vary depending upon your program funding stream.
