Required documents for each contract a provider has with The Community Partnership. Includes general deliverables, program rules by program type, and financial deliverables.
All providers are required to comply with the First Source Employment Program. The First Source Law requires all beneficiaries of contractual agreements totaling $300,000.00 or more are required to enter a First Source Employment Agreement with the Department of Employment Services. To enter this agreement, providers must register using the First Source Online Registration and Reporting System (FORRS).  FORRS will provide a more efficient way for companies and organizations to remain compliant with monthly First Source reporting requirements. Beginning January 2017, FORRS will be the sole registration and reporting system for companies responsible for managing and reporting First Source employee data.
Critical Incident and Unusual Incident Reports are used to report any unusual incident and actions taken by Employees, Contractors, Subcontractors and Volunteer at your program. Critical and Unusual Incidents should be reported in within 24 hours to TCP and the DHS (when applicable).
